Social Support Program

 
Objectives The objectives of the Social Support program are to:
• Offer a free, friendly, quality service to frail/aged/disabled people
• Assist as well as encourage people/residents with a disability who are isolated in their own homes
• Assist people to have a more active life in the community
• Enhance the quality of life for the client
• Facilitate and help the clients reach full active potential.
Eligibility Social Support is funded to provide services to frail aged/residents and people with a disability over the age of 65 living in Department of Housing accommodation in the Gladesville areas.
Social
Referral to the Service Social Support will accept referrals from medical practitioners, health professionals, service providers, friends and family members.
Services available
Social Support can provide a wide range of services:
• Individual transport to appointments
• Facilitate small groups
• Letter writing/letter reading
• Referrals for assistance in the home
• Bus trips
• Home visits to break the barrier of isolation.
Volunteers
Ongoing support and training will be provided to all volunteers.
Confidentiality and Security All information provided by clients is kept confidential.
